$3,000 sign on bonusThe Transitions Nurse is a registered professional nurse who is responsible for providing a comprehensive education, coaching and support to the chronically ill/high risk patients.  Provide both acute and community-based services.  Provides direction to enhance the effectiveness of the health care team through communication, teamwork and collaboration.  Monitor and manage transitions across physical settings (e.g. acute to community), and transitions in health status to improve patient care and outcomes. Responsible for managing an active caseload ranging from 20 to 25 patients, with an average of 20 patients. Support patient self-management and increase knowledge of his/her disease process by providing coaching and patient education. Increase patients ability to access timely and appropriate health care services, community resources and referrals. Provide patient information back to primary care providers. Responsible for establishing and maintaining programs that decrease the risk of 30 day-readmissions in chronic disease-state patients.Boulder Community Health (BCH) Primary Care Physician Clinics are guided by the philosophy of the Patient-Centered Medical Home. The medical home organizes care around patients, while engaging a multidisciplinary Care Team working proactively to coordinate and track care for each individual patient.Primary Responsibilities: Responsible for establishing and maintaining positive relationships and actively marketing to referral sources within the hospital, physician offices, or healthcare facilities Information resource for home health service/SNF services Resources for early intervention Assist complex patient conferencing as needed Daily rounds on hospital floors and morning meetings with all case managers to collaborate on patients identified as requiring homecare/SNF services Initiate interview process with new client/families Collect client information and signatures as needed Communicate with referral sources regarding services Serves as liaison between homecare/SNF and referral sources Keep records in automated system Seek appropriate medical and/or staff intervention in a timely manner to assure problem prevention and resolution Encourages patient self-management of health and works with patient to meet health and treatment goals, including providing education to patients regarding his or her specific care needs Adheres to best practices and evidence-based guidelines to provide patient-centric care Assess patients medical and psychosocial needs to develop, communicate and implement a comprehensive plan of care in collaboration with the healthcare team Document patient data, contacts and interventions in the medical record Create new and revises existing patient education resources and tools Track and trend transitional care patients and their care.  Reports weekly updates to Manager in writing Provides leadership and acts as a role model for staff; promotes a positive work environment. Employees are expected to comply with all regulatory requirements, including Joint Commission Standards Is familiar with organization, department and job specific Environment of Care areas, including Life Safety, Utilities Management, Hazardous Materials Communications, Emergency Preparedness, Infection Control and Medical Equipment Failure Adheres to Standard Precautions as appropriate, which may include: the use of protective barriers, as appropriate (e.g., gloves, masks, gowns, pocket masks, and/or safety glasses); handling and disposing of infectious waste appropriately; and hand washing as appropriateOther Responsibilities: Attend BCH meetings with Quality and Provider Relations staff Participate in meetings as needed and/or required Other duties as assignedDaily Practices: Knowledge of care plan formation Knowledge of community resources/agencies Knowledge of NCQA, QA, Medicare/Medicaid guidelines Knowledge of program and workflow development, process improvement and implementation Skilled in conflict management to assure problem prevention and resolution Skilled in developing and maintaining positive relationships and communicating effectively with internal staff and external customers Skilled in organization, coordination and facilitation of meetings and/or events Able to perform intermediate level of competence with various computer software applications including MS Outlook, Word, Excel, and Power Point Able to effectively communicate with internal staff and external customers Able to solve resource issues in a creative, positive manner to improve clinical outcomes of patients Able to accept and work with diverse populations and provide culturally sensitive coaching, education and assistance to patients/families Able to work with the community care management team members to most effectively manage members Able to participate in program development in order to identify appropriate goals of program Must be able to assess, evaluate, and respond to acute issues in a timely, competent manner Must be able to function with frequent interruptions, accept change and able to prioritize activities when faced with competing demandsYoull be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications: Associate degree RN (ADRN) or higher with an RN concentration Current Colorado Nursing license Requires valid Colorado Drivers License and insurance coverage 3+ years of case management experience or a combination of relevant experience 1+ year of work experience in ambulatory care setting, i.e. primary care clinic, urgent care, case management, home health care, or equivalent Experience with Care Transitions Coaching model You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role. Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.Preferred Qualifications: Experience managing psych/social issues with patients and families preferredUnitedHealth Group is an essential business.
