Make yourself at home with us and start doing your life’s best work.(sm)The Maternal Child Health Program Coordinator (MCH-PC) has strategic oversight, cross-functional management and responsibility for the delivery of quality and affordability results for the MN Health Plans Maternal Child Health Programs. The MCH-PC must have a current and unrestricted MN RN License and will be based at the XX UnitedHealthcare office or (designated a Telecommuter). Results are critical to this Program Managers success, as is a personal alignment with UnitedHealthCare mission to help Minnesotans live healthier lives and the core belief that an organization can manage cost and deliver quality and value to its customers. This position is responsible for ensuring the efficacy of the Maternal Child Health Programs through various internal and external collaborative relationships, continuous quality improvement strategies, and strategic initiatives.  The MCH-PC works closely with the Health Plan clinical leaders including the Chief Medical Officer, Health Services Director, and Quality Director to achieve the goals and objectives of the MN Maternal Child Health Programs.  It is ultimately the responsibility of the MCH-PC to oversee the primary clinical objectives by partnering closely with the Health Plan Leaders to ensure compliance with the state of MN vision for maternal and child health program initiatives. If you are located in Minnesota, you will have the flexibility to telecommute* as you take on some tough challenges. Primary Responsibilities: Clinical Operations Strategy Use industry knowledge of the XX maternity medical home and community-based partners landscape, to inform and execute on business objectives by developing the clinical operations strategy in partnership with the Community Stakeholder Leaders behind the maternal child health program Drive and lead translation of strategic objectives into relevant operational processes and measure for the maternity and pediatric populations Drive strategic growth and collaborative relationship development in the MN market, including development of strong partnerships in, community-based organizations, OB provider offices, hospitals, birthing centers, early childhood intervention programs, health systems, local health departments, and other partners Serve as an external facing liaison among various stakeholders supporting Maternity/NICU services to drive partnerships (e.g. public health departments, school-based health centers, behavioral health organizations, FQHCs/ACOs) and innovative strategies that will promote and enhance maternal and child health Clinical Operations Lead and manage the day-to-day operations of the Maternal Child Health Programs for MN Provide thought leadership, ongoing and annual evaluation of Maternal and Pediatric program design and efficacy as well as development of work plans to address performance improvement for the Health Plan  Monitor and oversee execution of performance and activities to improve health and quality outcomes and effective medical management (e.g., HEDIS, infant mortality, pre-term births, and NICU admissions) Promote preventive care and family planning services through various communication mediums and community-based events Identify and implement population health strategies (e.g. inter-conception care, chronic condition management, smoking cessation, safe sleep for infants, breast feeding advantages, substance use avoidance, etc.) In partnership with the Health Plan Leaders, develop and implement processes and programs to shape working relationships, understand provider, case management and community-based organization customer satisfaction, and support resolution of issues with internal and/or external stakeholders Monitoring, measuring and managing Health Plan performance against state compliance, quality and affordability goals for the Maternal Child Health Programs In partnership with the Health Plan, develop innovative approaches to improve engagement and overall program performance Partner with Compliance Lead on all compliance related topics, and ensure from a clinical operational perspective, there is compliance with all XX clinical case management and documentation requirements, policies, and procedures In partnership with the Health Plan Leaders, plan/implement corrective actions/solutions to address case management operational issues and concerns In partnership with the Health Plan Leaders, tracking practice-level HEDIS compliance and implementing strategic initiatives focused on timely closure of gaps in care and maximizing performance-based incentives Understanding contractual and NCQA delegation requirements and ensuring cross-functional staff are educated on, and follow them Designing and leading preventive health, quality improvement/performance improvement projects related to maternal child health Ensuring all components of the program, delivered by the Call Center Advocates, Optum CHWs and external providers and community-based organizations tie together into a comprehensive and seamless member experience In partnership with the Health Plan Leaders, create proposals for developing new initiatives or expanding existing programs as directed the state of XX. Work with internal and external partners to determine needed resources, staffing and operational execution steps as identified As warranted – Manage highly complex maternity or pediatric members as part of an inter-disciplinary care team approach in collaboration with Care Coordinators (CCs), Community Health Workers (CHWs), OB providers, behavioral health providers, etc. Youll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications: Current and unrestricted MN/Compact RN License 4+ years of Maternal and/or Pediatric clinical experience 3+ years of Care coordination and/or Case Management experience 2+ years of experience working in Medicaid 2+ years of leadership experience (e.g., Review work of others, Serve as an expert in their field, Serve as a mentor and leader, Lead functional or segment teams or projects, Motivate and inspire team members) Clinical program development and/or program oversight experience  Program management and evaluation experience, including tracking and trending of data as well as data analysis and strategic planning Proficiency with Microsoft Office Suite  Intra-state travel up to 25%Preferred Qualifications: Advanced degree (BSN, MSN, MPH) Community and/or public health experience Project management, change management experience, demonstrated history of managing programs or projects Experience or lived experiencing serving Black, Indigenous People of Color (BIPOC) Applying an equity lens and culture competency Excellent communication, writing and presentation skillsCareers at UnitedHealthcare Community & State.
