Title of Position: Integrated Behavioral Health Support Position Type: Part Time Pay Range: Starting pay is $38,480.00 (Base Pay $36,400.00 + Locale Diff $2,080.00 = $38,480.00) – $43,784.00, based on 1.0 FTE, annually and considers preferred experience and preferred education described below. Eligibility for Bonuses: No Benefits include: All Paid Leave Public Health Emergency Leave Health Insurance Dental Vision Short Term Disability Life Insurance 401k Location: Pagosa Springs, CO Description of Responsibilities: The purpose of the Integrated Behavioral Health Support (IBHS) position is to support behavioral health integration in the Community Health Center (CHC) Integrated Clinics. IBHS’ are an integral part of the clinic Care Team and work alongside the Medical Primary Care Providers (PCPs), Integrated Behavioral Health Providers (IBHPs) and other support staff. This position is responsible for engagement of clinic patients in behavioral health through population-based assessments and screenings on the CAT-MH or specific CAT-MH domains, and tracking patients’ follow-up and progress. Monitoring will be done using registries and the BH Care Coordinator facilitates changes in care for patients who are not improving in consultation with other Care Team providers. This position also assists the IBHPs by scheduling appropriate patients for treatment and contacting patients who do not show for appointments. Each IBHS position is responsible for two clinics, which will be covered by a combination of both in person and virtual. Supervisory responsibilities: No Physical demands: Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. AHS may make reasonable accommodations to enable individuals with disabilities to perform the essential functions. Travel required: Yes Required Education Licensure and Experience: Education: Bachelor’s Degree in Social Work, Psychology, Counseling, or equivalent field Experience: 2 years volunteer or paid experience in a healthcare or social service setting Must be knowledgeable regarding patient health information and HIPAA compliance. Must be organized and demonstrate the ability to follow guidelines. Must demonstrate excellent interpersonal and communication skills. Must demonstrate the ability to be self-directed. Must demonstrate the ability to follow the organization’s confidentiality and security rules. Required Skills: Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax and multi-line telephone. Working knowledge of Windows-based computer applications including, MS Edge, Excel, Outlook, Teams, Word, Office 365 and ability to access the internet. Ability to be helpful and sensitive to the needs of all employees, while concurrently respecting corporate policies, procedures and priorities. Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers. Ability to communicate job-related information to individuals and small groups. Ability to work independently with minimal supervision; plan own work schedule effectively, multi-task and prioritize in a fast-paced setting. Ability to multi-task, organize and prioritize workload in a sometimes-hectic environment with frequent interruptions. Ability to concentrate in sometimes hectic and stressful settings. Ability to read and understand written materials. Must regard work as confidential. Must have current driver’s license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with AHS’ insurance agency. Travel between clinics as required Ability to take on additional duties when necessary.
